We work very hard to ensure that our claims process is Simple and Transparent, and to ensure we pay out your claim as fast as possible. Complete your Death Claim using our online form, or download the necessary claim form for any other claims.

Death Claim
Supporting documentation required:
- Death Certificate
- Copy of Identity Document of Policyholder
- Copy of Identity Document of Deceased
- Copy of Identity Document of Beneficiary (If Main Life has passed away)
- Proof of banking
- DHA 1663
- Other documentation may be required at the time of assessing the claim
Cancer Claim
Supporting documentation required:
- Histology Report
- Copy of Identity Document of Policyholder
- Proof of banking
- Other documentation may be required at the time of assessing the claim
Unemployment or Retrenchment Claim
Supporting documentation required:
- Copy of Identity Document of Policyholder
- Retrenchment Letter
- Last working month’s Payslip
- Proof of continued Unemployment
- Proof of banking
- Other documentation may be required at the time of assessing the claim
Disability or Dread Disease Claim
Supporting documentation required:
- Copy of Identity Document of Policyholder
- Board or Medical Certificate on the Doctors Letterhead
- Proof of banking
- Other documentation may be required at the time of assessing the claim
